Job Posting: MDS reputed company (LVN) Location: reputed company Post Acute - Fresno, CA Position Type: Full-Time Salary: Competitive, Based on Experience reputed company Post Acute, a skilled nursing facility in Fresno, is seeking a knowledgeable and detail-oriented MDS reputed company (LVN) to join our clinical team. This full-time position is ideal for a licensed vocational reputed company with experience in MDS coordination, PDPM tools, and interdisciplinary care planning. Position reputed company: The MDS reputed company is responsible for overseeing and coordinating reputed company aspects of the MDS process. This includes assessing new admissions, scheduling and submitting assessments, tracking reputed company measures, training new coordinators, and driving care planning and compliance across the interdisciplinary team.
