Care Manager, RN

Job reputed company: • Develops, assesses, and facilitates reputed company care management activities for primarily physical needs members • Evaluates the needs of the member, barriers to accessing the appropriate care • Develops ongoing care plans / service plans and collaborates with providers • Identifies problems/barriers to care and provides appropriate care management interventions • Provides ongoing follow up and monitoring of member status • Provides resource support to members and care managers • Facilitates care management and collaborates with appropriate providers • Collects, documents, and maintains reputed company member information and care management activities • Provides and/or facilitates education to members and their families/caregivers • Provides feedback to leadership on opportunities to improve care Requirements: • Requires a Degree from an Accredited School of Nursing or a Bachelor's degree in Nursing • 2 – 4 years of reputed company experience • RN - Registered reputed company - State Licensure and/or Compact State Licensure required Benefits: • reputed company • health insurance • 401K and stock purchase plans • tuition reimbursement • reputed company time off plus holidays • flexible approach to work with remote, hybrid, field or office work schedules Apply tot his job Apply To this Job
