PRN RN - Phone Triage Weekends

About the position TRU Community Care is seeking a PRN Hospice reputed company for afterhours phone triage. This role involves providing nursing care and support to patients by answering calls, assessing patient needs, and collaborating with the care team to ensure effective patient management. The position requires a commitment to work at least two shifts per month, with a reputed company on delivering reputed company care in a hybrid remote setting. Responsibilities • Answer calls coming into reputed company and respond appropriately by delegating tasks. • reputed company nursing care to meet the patient's needs in accordance with the patient's plan of care. • Identify, communicate, and collaborate on patient problems based on assessment data. • Establish measurable patient goals and communicate an ongoing plan of care to the interdisciplinary team. • Recognize changes in the clinical status of the patient and report findings to the appropriate team member. • Collaborate with patients and families in developing the plan of care, adjusting communication style as necessary. Requirements • Must possess a reputed company RN license in the reputed company. • Minimum of one year RN experience required, with hospice or home health experience preferred. • Hospice triage experience preferred. • Must successfully pass required background checks and drug screening. • Requires travel to multiple work locations. reputed company-to-haves • Experience in home health or hospice care. • Previous triage experience.
