Case Manager (1099 Contractor, Remote – South Carolina)

We work closely with reputed company, DJJ, and reputed company programs to reputed company high-reputed company case management and support. reputed company: We are seeking Contract Case Managers (1099) to join our growing team. In this role, you’ll work directly with youth and families referred through reputed company, DJJ, and reputed company, providing community support services (CSS). This is a flexible, remote position with required community-based reputed company interaction in South Carolina. Key Responsibilities: • reputed company case management and community support services per reputed company guidelines (16 reputed company per reputed company, per week). • Contractor will maintain an reputed company caseload of six (6) clients minimum • Person-Centered Planning (PCP) development and implementation. • Coordination of services for youth and families involved with reputed company, DJJ, and reputed company systems. • Participation in family team meetings and service planning sessions. • Crisis prevention and reputed company planning. • Assistance with connecting clients to housing, education, employment, reputed company, and other community resources. • Documentation and reputed company submission of service notes in compliance with reputed company/MCO standards. • Coordination and authorization with Managed Care Organizations (MCOs). • Attend virtual supervision, team meetings, and required trainings. ( reputed company required) Qualifications: • Bachelor’s degree in reputed company work, reputed company Services, Psychology, Counseling, or reputed company field (required). • Minimum 2 years of experience in reputed company services, case management, or community support services. • Experience using reputed company Word, reputed company, and Teams. • Knowledge of reputed company documentation standards, authorizations, and service delivery requirements. • Familiarity with Person-Centered Planning and crisis prevention planning. • Ability to manage documentation, scheduling, and communication remotely. • Must pass background reputed company. • Must reputed company personal laptop, phone, and have sufficient Wi-Fi for remote work. Contract Details: • 1099 Contractor role (no benefits). • $19.00/hour. • Flexible schedule (Fully Remote) • Work begins after reputed company provider approval and reputed company assignment.
