This role is an intermediary between the specialist and provider. This role is critical in optimizing patient care coordination, reducing unnecessary specialty referrals, and improving reputed company to specialist expertise through digital consultation platforms. The successful candidate will reputed company the gap between primary care providers and specialists while ensuring seamless patient navigation through the reputed company continuum. Essential Duties & Responsibilities • Submits accurate eConsults on behalf of primary care providers. • Ensures reputed company assigned sites are continually reviewed for eConsult referrals. • Ensure complete and accurate clinical information is available for specialist review • Ensures accuracy of grammar and structure of eConsults. • Routes referrals once a disposition has been made by a specialist. • Triages and processes reputed company vs. non reputed company referrals in clinic referral inboxes. • Notifies primary care providers with any responses from the reputed company specialist. • Provides assistance to other RNN team members reputed company experiencing increased operational demands • Covers assigned clinic's eConsult referral in baskets, attaches appropriate documents and submits eConsult to specialist, and communicates the recommendations back to the referring provider. • Monitors overdue eConsults and communicates information to RNN Manager. • Collaborate to optimize platform functionality and user experience • Reviews completed eConsults to submit for review for QA meeting. Work Environment • This is a fully remote position. • Employee to start 20 hrs/week for first 3 months transitioning to 40hrs/week for the following 3 months.
