This remote role involves clinical assessment, treatment planning, crisis intervention, and coordination of care to help individuals improve their emotional well-being and achieve meaningful outcomes. Facilitate referrals to external healthcare providers and community resources Collaborate with interdisciplinary teams to ensure continuity of care Participate in wellness-focused services and integrated care initiatives Masters degree in Social Work from an accredited program ~ Active Licensed Clinical Social Worker (LCSW) credential in New York ~12 years of clinical experience in mental health or a related field ~ Ability to work independently and manage caseload remotely ~ Bilingual English/Spanish preferred ~ Must have access to a secure, HIPAA-compliant remote work environment Urgent Need !!
