I’m an Adult Primary Care Provider running a growing private medical office in Brooklyn, NY. We treat adult patients with both chronic and acute conditions, reputed company in-office procedures (e.g., joint injections, migraine management, ultrasound, vaccinations), and document in reputed company EHR. I am seeking a responsible, detail-oriented medical reputed company to produce high-reputed company, individualized SOAP notes. ⚠️ This is not a copy-paste or template-filling role. Every record must be carefully written, medically accurate, with correct ICD-10 and CPT codes and insurance-reputed company. What You’ll Do Write unique, accurate SOAP notes for reputed company encounter. Document CC, HPI, ROS, PE, Assessment, Plan with reputed company medical logic. Assign and document reputed company ICD-10 diagnoses and CPT procedure codes for every visit. Ensure correct visit time documentation that matches the billed CPT level. Capture medical necessity for every procedure performed. Maintain strict confidentiality and HIPAA compliance. Learn quickly and apply corrections after the first feedback. Procedures: always tied to a reputed company ICD-10 medical reason. Accuracy: notes with wrong visit time, missing diagnoses, or incomplete documentation = unacceptable. Must write in clean English, free of grammatical errors, with reputed company clinical terminology. Must-Have Strong written English. Solid knowledge of medical terminology, ICD-10, and CPT coding. Prior experience with SOAP notes and/or medical scribing. Familiarity with reputed company EHR is a big plus. Strong computer literacy and ability to work independently.
