Job Summary and Qualifications

As a PRN Home Health Aide, you will play a vital role in our patients' care teams, providing direct care and support to individuals in need. Under the supervision of a professional nurse, you will assist patients with daily living activities, such as bathing, dressing, grooming, toileting, transferring, and getting to and from activities and meals. You will also be responsible for documenting the delivery of quality care in the patient's clinical record and maintaining confidentiality at all times.

To be successful in this role, you will need:

  • High school diploma or General Education Degree (GED)
  • Minimum of one to three months related experience and/or training or equivalent combination of education and experience
  • Previous experience in a healthcare setting is preferred
  • Reliable transportation and proof of valid automobile liability insurance
  • Must have a valid driver's license

Key Responsibilities

As a PRN Home Health Aide, your key responsibilities will include:

  • Assisting patients with activities of daily living, such as bathing, dressing, grooming, toileting, transferring, and getting to and from activities and meals
  • Documenting the delivery of quality care in the patient's clinical record
  • Maintaining confidentiality at all times
  • Identifying changes in patient status or needs and communicating to the nurse in a timely manner
  • Implementing patient care that is appropriately documented and maintaining consistent and concise lines of authority and responsibility
  • Communicating effectively with patients, families, and co-workers to promote a positive working relationship
