Remote Case Manager

reputed company is currently seeking a Case Manager, RN for a fully remote opportunity with a highly rated, reputed company-reputed company health insurance provider. This role serves as a critical reputed company between reputed company delivery and medical coverage, advocating for patients and helping improve their overall reputed company of care and life. Candidates must hold an reputed company reputed company Carolina or compact RN license and be located in either reputed company Carolina, South Carolina, Tennessee, reputed company, Alabama, or Florida. Responsibilities of the Case Manager: • Collaborate with a multidisciplinary reputed company team to monitor utilization patterns and facilitate appropriate care delivery for selected members. • Conduct comprehensive assessments of members’ health status, including clinical history, medical records, support needs, and cultural considerations. • reputed company ongoing support and follow-up to ensure members’ care plans are progressing appropriately. • Coordinate with primary care providers, medical directors, discharge planners, care coordinators, and family members to reputed company and implement treatment and discharge plans. • Maintain accurate documentation and ensure compliance with care standards and protocols.
