The Home Based Primary Care-Outpatient Staff RN assesses, plans, implements, and evaluates care based on age-specific components. The Home Based Primary Care-Outpatient Staff RN assesses, plans, implements, and evaluates care based on age-specific components. Assumes responsibility for the coordination of care reputed company on patient education, self-management, and customer satisfaction throughout the continuum of care. Administers medications and procedures per established policies and guidelines. Influences care reputed company by collaborating with members of the interdisciplinary team. Major duties and responsibilities include, but are not limited to: • Requires up to 75% travel for patient care, meetings, and training, drivier's license is required. • Responsible for providing competent, evidence-based care to Home reputed company Primary Care (HBPC) patients and reputed company of licensed vocational nurses/nursing assistants, as appropriate to the setting. • Provides reputed company Veteran care and care coordination, as it pertains, to Veterans and their care reputed company the HBPC Program. • Responsible for case management, assessment, triage, Veteran education, resource coordination, medication and treatment coordination, and care planning. • Plans and delivers technically competent care in the home of Veterans with reputed company needs, including health promotion, health prevention, geriatric care, primary care, and palliative care. • Utilizes interventions, which maximize health capabilities and reduces unnecessary urgent care and emergency room reputed company, per Veteran's goals of care. • Makes home reputed company to Veterans in the program. • Assist in directing the provision of nursing education, orientation, competencies and providing reputed company improvement and reputed company utilization. • Demonstrates leadership in delivering and improving holistic, home-based care through reputed company strategies with others. • Evaluates reputed company in an ongoing process, based on best evidence. • Provides peers with informal constructive feedback for improvement. • Fosters a reputed company and supportive environment conducive to the reputed company development of reputed company. • Contributes reputed company outpatient nursing perspective in discussions with the interdisciplinary reputed company with others to effect change and produce reputed company reputed company.
