Remote RN Care Manager for Carceral Programs
JOB DESCRIPTION
Job Summary
Molina Healthcare Services (HCS) collaborates with members, providers, and a diverse team to assess, facilitate, plan, and coordinate a comprehensive delivery of care. This includes both behavioral health and long-term services for members with significant needs. Our team is dedicated to enabling patients to achieve their desired health outcomes through high-quality, medically appropriate, and cost-effective care.
We are looking for a qualified RN with an active WA state licensure to join our team in supporting the Washington State Plan. Ideal candidates will have knowledge of the criminal justice system, or experience within carceral settings. Previous experience in case management, discharge planning, or care coordination is essential. This is a remote position.
Work schedule: Monday - Friday, 7:30am - 4:30pm PST.
RN WA licensure required.
KNOWLEDGE/SKILLS/ABILITIES
• Conduct thorough and timely assessments of members to identify eligibility for case management based on clinical evaluations and psychosocial factors.
• Collaborate in developing and implementing comprehensive case management plans with members, caregivers, physicians, and necessary healthcare professionals to address individual needs and goals.
• Perform face-to-face or home visits as dictated by member needs.
• Monitor care plans continuously, documenting interventions and tracking goal achievement, adjusting plans as necessary.
• Maintain an active case load for ongoing member outreach and management.
• Facilitate integration of services, including behavioral health and long-term supports to enhance seamless care for Molina members.
• Coordinate interdisciplinary care team meetings and foster informal collaboration among team members.
• Utilize motivational interviewing techniques and Molina's clinical guidelines to educate and inspire members toward positive health changes.
• Identify barriers to care and provide necessary coordination to assist members in overcoming these challenges.
• Local travel required: 25-40%.
• Offer consultation, recommendations, and educational support to non-RN case managers.
• Manage complex cases involving members with intricate medical conditions and medication regimens.
• Conduct medication reconciliation as needed.
JOB QUALIFICATIONS
Required Education
Graduate from an accredited School of Nursing; Bachelor's Degree in Nursing is preferred.
Required Experience
1-3 years in case management, disease management, or in medical or behavioral health settings.
Required License, Certification, Association
Active, unrestricted State Registered Nursing (RN) license in good standing.
Must possess a valid driver's license with a good driving record and have reliable transportation for local travel.
Preferred Education
Bachelor's Degree in Nursing.
Preferred Experience
3-5 years in case management, disease management, managed care, or medical or behavioral health settings.
Preferred License, Certification, Association
Active, unrestricted Certified Case Manager (CCM).
