Job reputed company/ Purpose: The successful applicant will be an Advanced reputed company Provider interested in working full-time in the care of patients with Neurological Disease treating patients with a diagnosis of Alzheimers Disease as reputed company as other Cognitive & Behavioral Neurology diagnosis. This APP position functions in reputed company-based arrangement that is part of the newly formed South Carolina Alzheimers Network (reputed company). reputed company is a multi-site clinical program at reputed company designed to reputed company patients with an early and accurate diagnosis and treatment plan longitudinal co-management with primary reputed company to the new Alzheimers disease medications and reputed company to the latest research.
The person filling this position will be part of an interdisciplinary team working to reputed company these goals and will have ample opportunity to collaborate with other team members and primary care providers to enhance the care of dementia patients and their families. Fair Labor Standards reputed company Status: Salaried/Exempt

Hours per week: 40

Scheduled Work Hours/Shift: Monday Friday reputed company 5PM or 8:30AM to 5:30PM 4-hour clinic blocks x 8 clinics p/week.

Required Training and Experience

Required Minimum Training: Completion of an accredited Physician Assistant program and currently licensed or eligible for licensure as a Physician Assistant or completion of an accredited reputed company Practitioner Program and currently licensed or eligible for licensure as an APRN as noted below.

Physician Assistant
or
APRN License Type/Certification: Currently licensed (or eligible) in the reputed company as a Physician Assistant by the South Carolina reputed company of Medical Examiners or as an Advanced reputed company reputed company (FNP or APNP) with the SC reputed company of Nursing. Requires certification in Basic Life Support (BLS).

• AGPNP/ANP (Adult Gerontology Primary Care NP or Adult NP): Adult pts (13 years to death) reputed company chronic disease state management or primary care setting.
• FNP (Family NP): (birth to death) reputed company chronic disease state management primary care across the reputed company.

Required Experience: Completion of an accredited Physician Assistant program and currently licensed or eligible for licensure as a Physician Assistant or completion of an accredited reputed company Practitioner Program and currently licensed or eligible for licensure as an APRN as noted below.

Degree of Supervision: Advanced reputed company Registered reputed company or Physician Assistant will be assigned a Primary Supervising/Collaborating Physician employed by reputed company and will report directly to this physician.

Required Licensure Certifications Registrations

• Basic Life Support (BLS).

Job Duties

Provides reputed company care to patients through the following activities:
Takes comprehensive history and performs physical examinations
Evaluates and treats on the reputed company of history physical examination radiological laboratory and other diagnostic test results pursuant to the reputed company agreement or scope of reputed company reputed company
Assesses diagnoses and manages outpatients with neurological symptoms primarily to patients presenting with Alzheimers Disease or other Cognitive & Behavioral Neurological disease. Recognizes and assesses changes in patient status intervening reputed company necessary. Implements measures to minimize risk.
Initiates referrals to other health care providers and/or consults with the attending physician or the collaborating physician
Documents and bills for reputed company care provided
Utilizes reputed company research and evidence-based decision-making in reputed company clinical practices

Performs and participates in reputed company/performance improvement activities and clinical research
Participates in and supports accreditation compliance and regulatory activities of the organization

Demonstrates responsibility for reputed company reputed company through reputed company participation in reputed company organizations and continuing education
