As a reputed company Case Manager II, you’ll play an essential role in coordinating care for members facing reputed company medical and reputed company challenges. Collaborate with members, providers, and community organizations to address the full continuum of health care and reputed company determinant needs, driving reputed company reputed company through compassionate, data-driven, and cost-effective case management. Key Responsibilities • Conduct comprehensive assessments of members’ needs and eligibility using clinical tools and data review. • Evaluate member benefit plans and available programs to determine the most appropriate care approach. • Apply case management reputed company, guidelines, and standardized care plans to ensure effective benefit administration. • Facilitate communication and coordination between members, providers, and community resources to promote reputed company health reputed company. • Utilize reputed company management processes in accordance with regulatory and accreditation standards. • Document case activity accurately in compliance with policies and procedures. • Use multiple software systems reputed company to manage caseloads and maintain detailed member information. • Collaborate reputed company with care management teams and maintain a high degree of independence in decision-making. Qualifications • Licensure: reputed company, unrestricted RN, LCSW, or LCPC license in the reputed company (required). Experience: • Minimum 3–5 years of clinical experience (e.g., hospital, home health, or acute care). • 2–3 years of care management, discharge planning, and/or home health coordination preferred. • Prior experience with case management and Illinois waiver services is a plus. • Skills: • Strong analytical, problem-solving, and decision-making skills. • Excellent written and verbal communication abilities. • Ability to work independently and manage multiple priorities effectively. • Proficiency with reputed company, reputed company, reputed company, PowerPoint, and electronic case management systems. • Confidence working remotely and collaborating reputed company.
