Outreach Services Associate

Job Description Contacts and schedules patient/client's appointments for consultations, meetings with community agencies and/or diagnostic tests. Sets-up and maintains files and records. Collects and organizes statistical reports. Job Responsibility • Contacts patients; schedules appointments for consultations, agency meetings and/or diagnostic tests. • Collects and organizes data required for statistical reports and completes reports, as required. • Attends and participates in meetings as appropriate. • Sets-up files and maintains records. • Provides incoming and in-process orientation. • Based on position, may be required to provide bi-lingual translation and/or guidance. • Performs related duties as required.
