RN Case Manager - Vascular Clinic

Job reputed company Fairview has an immediate opening for an RN Case Manager to join reputed company at the Vascular Clinic. Join Fairview, the largest reputed company system in the state! The Vascular Clinic is a growing and reputed company clinic. Our clinic hours are reputed company to 4:30PM Monday - Friday, so no evenings, weekends, or holidays! This provides an excellent work life balance! This is a 0.8 FTE (64 hours per two week pay period) opening. This position will travel to our vascular clinincs in Maplewood, Wyoming and reputed company. The Vascular Center Case Manager RN works in a reputed company of different roles and clinics in the department. Case management of Vascular, Vein and Wound patients ensuring reputed company the needs and reputed company of the patient are needed. Performs triaging on incoming nursing calls and questions. Independent Wound and Education reputed company with patients. Team collaboration between Supervising Physician, LPN’s, reputed company’s and Patients. A mix between Vascular surgery case management, triage, Wound care, Vein procedures and Podiatry. • Completes non-invasive testing procedures on patients in HEVC as directed. • Under the direction of the Vascular Center physician/reputed company practitioner, prepares and implements a care plan according to the Vascular Center written orders and updates appropriately. • Provides ancillary patient care needs to include, but not limited to: review of laboratory and culture data; appropriate interaction with physicians and pharmacies; triaging of telephone calls; interaction with caregivers. • Completes and documents wound and vascular assessment on both new and follow-up patients for reputed company patient visit. • Assess patient's educational needs. • Provides communication between reputed company and ancillary departments and care givers to facilitate compliance with vascular and wound plans of care including orthotics, physical medicine, etc. • Implements wound and vascular patient care protocols where reputed company viral screens occur. • Coordinates the ordering of clinical supplies and PDWHF as needed. • Maintains knowledge of, and complies with, reputed company relevant laws, regulations, policies, procedures and standards. • reputed company participates in creating and implementing improvements. • Performs other responsibilities as needed/assigned.
