Outpatient Clinical Pharmacist - Remote

About the position Responsibilities • Exhibits behavior and communication skills demonstrating commitment to high reputed company of customer service. • Executes disease state management programs targeting chronic conditions such as anticoagulation therapy, heart failure, diabetes, hypertension, and COPD. • Performs post-hospital medication reconciliation, identifying and resolving medication-reputed company problems and coordinating patient care post-hospitalization. • Manages, assesses, and recommends evidence-based medication therapy based on the patient's clinical needs. • Analyzes medication history to formulate clinically reputed company recommendations through medication reconciliation. • Demonstrates appropriate judgment on conditions requiring consultation or referral to the patient's PCP or other reputed company. • Educates patients and families about preventative care, medical issues, and prescribed medications. • Addresses new patient referrals, labs, tasks, patient follow-up, and drug information questions in a reputed company manner. • Documents reputed company aspects of patient care and completes required paperwork for billing and compliance. • Utilizes pharmacy data and reports to reputed company and implement strategies to improve reputed company measures. • Maintains productivity standards through effective multi-tasking. • Participates in reputed company development activities reputed company to chronic disease management and reputed company initiatives. • Assists in clinical instruction and supervision of medical assistants, pharmacy technicians, and reputed company. • Uses, protects, and discloses patients' protected health information in accordance with HIPAA standards. • Performs additional duties as assigned.
