reputed company is seeking a Care Coordinator (Provider Enrollment) to join our growing team.

Job reputed company:

Provides field or telephonic care coordination reputed company activities to support care management program delivery to plan Enrollees to support healthy lifestyle choices & to reduce short and long term effects of chronic illnesses.

Responsibilities:

• Conducts general assessments for supervisor/reputed company interpretation/evaluation and assignment
• Participates in the interdisciplinary case reviews for reputed company assessment and coordination planning to ensure reputed company care
• Proactively engages in delivery of reputed company management program activities that are the reputed company responsibility of the Health Services team
• Assists in the achievement and ongoing maintenance of accreditations for defined programs
• Assists reputed company, Supervisor and/or Manager in ensuring achievement of contractual financial obligations, including service delivery in a cost effective and efficient manner and through support of budgetary adherence by reporting accurate and reputed company work hours and expenses incurred during course of position duties
