Department & Position Overview The Structured Family Caregiving program is a unique caregiving arrangement where a patient lives with a principal caregiver who provides daily support based on the patient's needs. As a Case Manager - Health Coach, you will work closely with waiver members, caregivers, and stakeholders to provide a seamless customer service experience, ensuring clients receive the necessary care. Key Responsibilities: • Schedule and perform initial home assessments (virtual and in-person) to understand the needs of waiver members and caregivers. • Serve as a field expert to support waiver member and caregiver onboarding, ensuring a smooth and informative experience. • Conduct required "home inspections" to ensure the safety and compliance of participants' homes and surroundings. • Partner with the care team to monitor daily notes submitted by caregivers, identifying potential training or support needs. • Maintain regular contact with caregivers and waiver members, providing guidance and support as needed. • Collaborate with the care team to transition newly onboarded waiver members to the ongoing program phase. • Communicate any concerns regarding waiver members' or caregivers' health, safety, or unmet needs to staff RN. • Track all activities through entries into CRM (Salesforce). • Function effectively in a fast-paced startup environment, adapting to changing priorities and deadlines. Ideal Candidate We're looking for a candidate with: • A bachelor's degree or equivalent work experience in a related field. • Two+ years of experience working with frail or elderly disabled adults and caregivers. • 2-3 years of experience in inside sales, call center, telephonic outreach, customer service, or a similar field. • 1-3 years of experience in the medical field, case management, or home health. • Strong verbal and written communication skills, with the ability to establish rapport and effectively communicate with waiver members and caregivers. • Excellent problem-solving skills, with the ability to identify issues and escalate when necessary. • Strong time management, critical thinking, and active listening skills. • A high level of empathy and emotional intelligence. • Proficiency in computer software navigation, with experience in Salesforce a plus.
