Located on a beautifully landscaped 27-acre medical campus at 19339 Benn’s Grant Boulevard in Smithfield, the new hospital will provide modern facilities and comprehensive medical services to meet the growing needs of the community. The 200,000-square-foot, three-story facility will offer a range of essential healthcare services including an Emergency Department, Diagnostic Imaging Services, 38 Medical/Surgical beds, 12 Intensive Care Unit (ICU) beds and 4 Operating Rooms. We anticipate start dates to begins Fall, 2025. This is a hybrid position acting as a Total Joint Nurse Navigator and Pre Surgical Assessment Nurse. Overview Responsible for the assessment, preparation, and education of surgical and procedural patients. The Total Joint Navigator utilizes communication, critical thinking, problem solving, and decision making to create a plan of care for each patient based on physiological, psychosocial, and cultural diversity. Works with total joint replacement patients and collaborates with the interdisciplinary team to guide, assist and transition patients/families through the joint replacement continuum. Provides leadership for the Hip and Knee Replacement Program and assists with standardization of processes across the care continuum. Facilitates education and staff development for total joint replacement. What you will do: Pre Surgical Assessment RN • Patient assessment and evaluation • Conduct comprehensive nursing assessment including medical history, medication review and allergy verification • Review diagnostic results (labs, EKG, Chest-xray, ETC) and identify potential perioperative risks. • Collaborate with anesthesia providers and surgeons for medical clearance. • Care Coordination • Facilitate required consults (cardiology, endocrinology). • Ensure timely scheduling and completion of preoperative testing. • Communicate patient readiness and special considerations to the surgical and anesthesia teams. • Patient Education • Provide education on surgical preparation, NPO guidelines and medication management. • Act proactively to reduce same-day cancellations and complications. Total Joint RN Navigator • Demonstrates ability to assess patient clinical and psychosocial needs by applying standards of practice as it relates to the specialty or patient population served. Completes preoperative screening calls and appointments completely and accurately. Assesses patient condition, history, and needs via chart review, telephone and/or in person in order to ensure optimization for procedure/surgery. • Appropriately identifies significant findings and demonstrates a sense of urgency to escalate these findings to the appropriate member of the patient care team in a timely manner. Ensures significant issues are followed up on appropriately and in a timely manner and escalates to the leadership team as needed. Organizes and plans daily activities in order to balance quality care with efficient use of time and resources. Accurately completes all required clinical and quality documentation consistently per RHS standards and regulatory requirements. Completes pre-assessment screening documentation in the electronic medical record in a timely manner. Clearly and accurately documents interactions with patients and caregivers throughout the pre-assessment process. Documents findings that require anesthesia review in a clear, concise manner to ensure effective communication with the anesthesia provider team. • Collaborates with providers on patient needs, barriers and assessment changes to include understanding their individualized diagnosis, comorbidities, medications, procedures and rehabilitation. Collaborates with physicians, advanced practice providers, nursing leadership, therapy staff and support services to facilitate and promote improved patient outcomes. Works with specialty program patients and collaborates with the interdisciplinary team to guide, assist and transition patients/families through the treatment continuum. Serves as the lead for pertinent collaborative committees and attends and other quality and committee meetings as appropriate. • Facilitates PI activity for the total joint program. In collaboration with organization leaders, prepares the organization for the accrediting body certification review survey. • Educates patients and their caregivers by assuming responsibility to individualize and coordinate patient education regarding pre- procedure/pre-op instructions. Provides patient and/or caregivers with clear, detailed instructions regarding preparation and arrival for their procedure/surgery as well as what to expect on the day of their procedure and during their immediate recovery period. Evaluates for patient and/or caregiver comprehension and documents accordingly. Responds to patient inquiries related to their pre-procedural/pre-op instructions. Contacts the identified surgical population, including but not limited to the joint replacement patients, post-surgery at various intervals through recovery to reinforce discharge instructions, medications, rehabilitation/recovery plans and adherence to follow up appointments. Provides leadership for the Hip and Knee Replacement Program and assist with standardization of processes across the care continuum. Represents the Hip and Knee Replacement Program on various hospital committees, as needed. Facilitates total joint replacement education and staff development. • Partners with patients and families to develop, implement and evaluate plan of care, treatments and procedures that contribute to the physiological and psychosocial outcomes of the patient. Assesses and incorporates evidence-based standards of practice. Collaborates with interdisciplinary team in the coordination care. Communicates regularly and effectively with physician practices to coordinate patient care. Facilitates safe patient transitions to the next care setting as part of plan of care by ensuring all elements of the pre-assessment screening process are completed prior to day of surgery/procedure. Communicates unique and/or outstanding patient needs to the preoperative team to ensure a safe, seamless transition of care on the day of surgery/procedure. Monitors patients in as needed populations to meet Quality Program requirements. Ensures facility meets quality reporting program requirements for total joint replacement procedures. • Evaluates for patient and/or caregiver comprehension and documents accordingly. Responds to patient inquiries related to their pre-procedural/pre-op instructions. Contacts total joint replacement patients post-surgery at various intervals through recovery to reinforce discharge instructions, medications, rehabilitation/recovery plans and adherence to follow up appointments. Provides leadership for the Hip and Knee Replacement Program and assist with standardization of processes across the care continuum. Qualifications Education • Nursing Diploma (ADN or BSN) or Program Graduate Required. If Program Graduate, High School Diploma will be required. Experience • 2 years Registered Nurse experience (Required) • 1 year Experience in Surgical Services or Orthopedic setting (Preferred) Skills and Abilities • General clerical skills, to include the ability to use the computer, copier/scanner/fax machine, conference phones, etc. • Microsoft Office knowledge, including Word, Excel, PowerPoint, Outlook • Precise communication skills, written and verbal • Problem solving skills • Organizational skills, with the attention to detail • Time management skills Licenses and Certifications • Registered Nurse (RN) - Virginia Department of Health Professions (VDHP) or licensed in a compact state Upon Hire (Required) • CPR/BLS Certification - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) Upon Hire (Required) • ACLS Provider - American Heart Association/American Red Cross/American Safety and Health Institute (AHA/ARC) within 90 Days (Required) To learn more about being a team member with Riverside Health System visit us at https://www.riversideonline.com/careers.
