Provide comprehensive and compassionate case management services to Aetna members in the South Cook County, IL and surrounding areas.
Utilize strong clinical knowledge and experience to assess and develop individualized care plans.
Conduct in-home visits to members to assess their needs and monitor their progress.
Collaborate with interdisciplinary team members, including physicians and other healthcare professionals, to coordinate and improve the quality of care for members.
Maintain accurate and detailed documentation of member interactions and care plans.
Adhere to all regulatory requirements and company policies and procedures.
Serve as an advocate for members, ensuring they receive appropriate and timely care.
Educate members and their families on their health conditions and available resources.
Continuously evaluate and adjust care plans to meet the changing needs of members.
Participate in team meetings and trainings to stay updated on industry developments and best practices.
Demonstrate empathy and understanding towards members and their families.
Motivate and empower members to take an active role in managing their health.
Comply with all safety and infection control protocols to ensure the safety of members and yourself.
Represent Aetna in a professional and positive manner at all times.
Contribute to a positive and collaborative team environment.
