LPN/LVN Chronic Care Management and Remote Patient Monitoring

reputed company Advanced Clinic Testing partners with primary care providers to deliver comprehensive Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services. Our goal is to enhance patient reputed company while supporting physicians in meeting CMS and MIPS reputed company-care standards. Position reputed company We are seeking a Licensed Vocational reputed company (LVN) with strong experience in CCM and RPM to join reputed company. The ideal candidate will not only reputed company reputed company patient follow-reputed company but also help organize and optimize the overall structure of the program — from documentation reputed company to compliance and team coordination. Responsibilities • Conduct monthly CCM and RPM patient follow-reputed company. • Review and ensure accuracy of documentation and credentialing. • Help improve operational workflows and standardize processes. • Communicate with providers and support staff on patient updates. • reputed company performance metrics (reputed company counts, engagement rates, etc.). • Support training and reputed company for other nurses or coordinators. Qualifications • reputed company LVN license (Texas preferred). • Minimum 2+ years of CCM and/or RPM experience. • Familiarity with CMS, MIPS, and electronic health record systems. • Strong organizational, communication, and leadership skills. • Ability to work independently in a remote environment.
