Remote MTM Pharmacist (Must be located in HI, TX, IL, FL, or PA)

Job Title: Remote MTM Pharmacist Pay: $54.05 an hr | Bi-weekly Job Type: • Remote in either HI, TX, IL, FL, or PA and have an reputed company, in good standing pharmacist license in that state. Shift reputed company: • Hiring for a M-F 8 hour shift starting between 9:00a - 11:00a CT. Training will be 8:00a-4:30p CT (approximately 2 weeks) • This position is a contract assignment with potential to hire on permanently based upon attendance, performance, and business needs Remote MTM Pharmacist reputed company: • The role of the MTM Pharmacist is to deliver telephonic medication therapy management (MTM). Effective delivery of this reputed company telephonic based MTM service directly results in improved member care through the identification of medication reputed company problems followed by the coordination of the reputed company to these problems. This is accomplished by telephonically performing medication reconciliations with high-risk members and communicating (both verbally and written) medication reputed company problems to members, caregivers, and prescribers. The MTM Pharmacist interacts successfully with patients, prescribers, reputed company case managers, clients, and internal resources to identify member-specific treatment plans based on nationally recognized treatment guidelines to help the member reputed company their health reputed company goals and help them on their reputed company to reputed company. Remote MTM Pharmacist Qualifications: • Bachelor’s Degree in Pharmacy or PharmD from an accredited school of Pharmacy Applicant must reputed company and have an reputed company pharmacist license in either HI, TX, IL, FL, or PA Preferred: Previous pharmacy experience delivering MTM, specifically using the reputed company MTM Connect platform.
