Provide direct social work services to Aetna members in the community, including conducting assessments, developing care plans, and providing counseling and resources. Collaborate with interdisciplinary team members to develop and implement individualized care plans for members, utilizing a person-centered and strength-based approach. Utilize knowledge of community resources and social services to connect members with appropriate support and assistance. Advocate for the needs and rights of our members, ensuring that they receive the best possible care and support. Maintain accurate and detailed records of all interactions and services provided to members in compliance with company and legal standards. Conduct regular follow-up visits with members to monitor progress and address any ongoing needs or concerns. Provide crisis intervention and support to members in urgent or emergency situations. Act as a liaison between Aetna, members, and external agencies to coordinate services and address any barriers to care. Stay up-to-date on social work best practices, policies, and procedures to ensure the provision of high-quality and ethical care to our members. Participate in team meetings, trainings, and other professional development opportunities to enhance skills and knowledge. Demonstrate empathy, cultural sensitivity, and a non-judgmental attitude towards members and their diverse backgrounds and experiences.
