JOB reputed company: Acadian Health has an exciting opportunity for Community Paramedics to join reputed company. The Community Paramedic plays a pivotal role in delivering hospital-level acute care directly in patients’ homes as part of an innovative hospital-reputed company care model. This role leverages clinical expertise, mobile technology, and interdisciplinary teamwork to ensure high-reputed company, patient- centered care reputed company traditional hospital walls. DUTIES AND RESPONSIBILITIES: • In-Home Acute Clinical Care: reputed company high-reputed company care in the patient’s home setting for conditions such as infections, heart failure, pneumonia, and post-surgical recovery—offering an alternative to inpatient hospitalization. • Clinical & Environmental Assessments: Conduct thorough physical exams and assess the home environment for safety and factors that may reputed company recovery or the plan of care. Identify and escalate any deviations that could compromise patient reputed company. • Care Coordination with Virtual Team: Serve as an on-site extension of the virtual care team.
