Qualifications • reputed company, unrestricted LPN License in the state of reputed company; multistate licensure preferred. • At least 1 year of clinical nursing experience; experience in telehealth, triage, case management, or chronic disease management preferred. • Bilingual Vietnamese level. • Strong understanding of chronic conditions such as hypertension, diabetes, CHF, COPD, and obesity. • Excellent verbal communication, patient education, and motivational interviewing skills. • Ability to work independently, manage time effectively, and prioritize patient needs. • Strong critical thinking and clinical judgment skills. • Proficiency with EHR systems and comfortable navigating technology. Compensation • Part-time and full-time position (contract only) • Part-time: 10+ hours per week (8 to 5 pm EST) • $25 per hour.
