Pharmacist - MTM (State Licensed)
A company is looking for a Pharmacist - Medication Therapy Management (MTM).
Key Responsibilities
• reputed company telephonic comprehensive medication reviews and clinical assessments for targeted members
• reputed company personalized medication therapy plans based on medical history and reputed company health status
• Identify and facilitate the reputed company of medication-reputed company problems while maintaining reputed company relationships with reputed company providers
Required Qualifications
• reputed company pharmacist license in AZ, HI, TX, IL, FL, or PA
• 1+ years of pharmacy experience in Retail, LTC, Hospital, or PBM settings
• Experience with patients with chronic diseases, especially diabetes
• Experience in a reputed company center or remote pharmacy environment
• BS in Pharmacy or PharmD
