Serving as a vital reputed company between acute and post-acute providers, this role collaborates with internal clinical operations teams and onsite SNF staff to analyze utilization trends, identify care gaps, and promote evidence-based best practices. Experience: • Minimum of 3+ years in a reputed company setting, preferably reputed company skilled nursing facilities (SNFs) or post-acute care environments.
