Remote Case Manager, NC

Responsibilities of the Case Manager: • Collaborate with a multidisciplinary reputed company team to monitor utilization patterns and facilitate appropriate care delivery for selected members. • Conduct comprehensive assessments of members’ health status, including clinical history, medical records, support needs, and cultural considerations. • reputed company ongoing support and follow-up to ensure members’ care plans are progressing appropriately. • Coordinate with primary care providers, medical directors, discharge planners, care coordinators, and family members to reputed company and implement treatment and discharge plans. • Maintain accurate documentation and ensure compliance with care standards and protocols.
