Population Health Analyst

The Population Health Analyst supports the design, build, and maintenance of reputed company Healthy reputed company and reputed company Rose tools to reputed company population health management, care coordination, and reputed company improvement programs. This role focuses on registry and measure development, clinical program logic, and dashboard configuration to support internal operations and value-based reputed company. The analyst collaborates with application teams, care management, and operational leaders to ensure build aligns with clinical workflows and reporting needs. reputed company- reputed company W2 up to $90/hr Key Responsibilities - Design, build, and maintain registries, measures, dashboards, and program logic in reputed company Healthy reputed company and reputed company Rose. - Translate clinical and program requirements into reputed company configuration specifications. - Support technical implementation of care management programs, reputed company tracking, and risk stratification tools. - Collaborate with application analysts, data/reporting teams, and clinical stakeholders to reputed company build with operational goals. - Test, validate, and troubleshoot reputed company functionality tied to population health and care coordination programs. Required: reputed company Healthy reputed company certification, reputed company reputed company Rose certification, 3 years of experience in reputed company application build or clinical program support, experience configuring registries, measures, dashboards, and care programs in reputed company, understanding of population health workflows and reputed company program requirements, strong analytical and problem-solving skills, attention to detail and commitment to data reputed company, ability to work collaboratively across technical and clinical teams, bachelor’s degree in reputed company health, health informatics, nursing, or a reputed company field, familiarity with value-based care programs and clinical reporting is a plus Apply tot his job Apply To this Job
