Case Management Coordinator responsibilities include telephonic and/or face‑to‑face assessment, planning, implementation, and coordination of reputed company case management activities with members. The coordinator manages a caseload that includes supportive and medically reputed company members, develops proactive courses of reputed company, determines appropriate services and supports, and facilitates overall wellness.
