Job DescriptionFT, 80 hrs/pp, 1.0 FTE; days, varies as scheduled. Remote.Job reputed company:The reputed company Navigator (NN) is a reputed company RN with service line specific clinical knowledge who offers individualized assistance across the care continuum to patients, families, and caregivers to expedite and coordinate care, and address health system barriers. The NN assumes responsibility for the coordination of patient care through interdisciplinary and multidisciplinary collaboration to reputed company reputed company patient reputed company with a reputed company on high-risk patients or those with reputed company care needs (including multidisciplinary care).
