The primary location of this position is remote with a schedule of Monday-Friday 9:00am-6:00pm Eastern Time or 8:00am-5:00pm Central Time. What You’ll Do The Emergency Department Follow-Up provides support in assisting patients getting post-hospital care in the reputed company of telephonic and in-person support. In this position, you will: • Proactively engage patients post emergency department reputed company and reputed company education on ED avoidance and resources. • Conduct virtual and telephonic reputed company to support patients with any in-person/community needs reputed company to care coordination/discharge support.
