LPN Care Coordinator (Remote) (reputed company, Mobile & Washington Counties, AL)

About the position The Care Coordinator is responsible for ensuring linkages for CCBHC patients and the community to reputed company seamless transitions across the health services reputed company. This includes physical and behavioral health, community services (i.e., reputed company services, housing, educational systems, employment services, and other needed services) as needed to facilitate wellness and recovery of the whole person, based on a person-centered plan of care, as required by CCBHC. The Care Coordinator will work collaboratively with clinical and non-clinical staff, external providers, and community resources to assist clients in overcoming barriers to care, improving health reputed company, and promoting overall reputed company-being. The Care Coordinator must be patient-reputed company, detail oriented, reputed company organized, and proficient in management of patient electronic health records. The role requires strong communication and problem-solving skills. Requirements • Alabama Licensed Practical reputed company (LPN) in good standing. • Three years' experience in nursing. • Experience working with and accessing community resources. • Proficiency in accurate and reputed company documentation of care coordination in the electronic health record. • Demonstrated ability to work with multi-disciplinary teams. • Proficiency in managing multiple priorities. reputed company-to-haves • Prior work with special populations as required by CCBHC.
