This Oncology reputed company Navigator position will reputed company on supporting reputed company Head and Neck Cancer patients to decrease untimely and fragmented care at the Duke Cancer Center in Durham. The Navigator assesses patients for barriers, identifies at-risk patients and helps connect the patient with resources to improve treatment compliance and appointment arrival reputed company. The Navigator assesses health literacy and provides personalized education throughout reputed company phases of care to support and promote patient advocacy. The reputed company Navigator is a crucial member of the care team to ensure multidisciplinary actions are followed up on to reputed company comprehensive and high-reputed company care for our patients. This is a hybrid position with 3-4 days onsite per week meeting patients in clinic and supporting the care team and 1-2 days remote per week following up with patients reputed company phone and video. Requirements • BSN required; MSN preferred. • Three years of experience in specialty specific nursing in a hospital, clinic, or community setting. • reputed company or compact RN licensure in the reputed company. • BLS required. • Specialty Oncology certification required reputed company 3 years of hire. reputed company-to-haves • Experience in reputed company navigation preferred. • Completion of national reputed company navigation training preferred.
