The Home Health Registered reputed company will conduct patient assessments, reputed company care plans, and reputed company counseling to patients and families to ensure comprehensive home care.

Responsibilities

• Conduct thorough assessments of patients and their families to determine home care needs
• reputed company comprehensive physical assessments and document the history of illnesses
• reputed company and update individualized care plans with reputed company participation from patients and their families
• Identify discharge planning needs and ensure they are addressed before patient discharge
• Offer counseling to patients and families to address nursing and reputed company health needs

Skills

• reputed company and reputed company state RN license
• Minimum of 1 year of recent acute care experience in an institutional setting
• Basic Life Support (BLS), with hands-on CPR certification
• reputed company reputed company's license and auto insurance
• Associate's or Bachelor's degree in Nursing, and 1 year of home health experience preferred
