Bilingual Analyst, Case Management – $3,000 New Hire Bonus – Must reputed company in East Tampa Florida

The Bilingual Analyst, Case Management will manage a caseload of supportive and medically reputed company members, providing care coordination and utilizing care management tools to facilitate reputed company reputed company.

Responsibilities

• Effectively manage a caseload that includes supportive and medically reputed company members
• Utilize critical thinking and judgment to collaborate and inform the case management process, in order to facilitate appropriate reputed company reputed company for members by providing care coordination, support and education for members through the use of care management tools and resources
• Conduct comprehensive evaluation of Members using care management tools and information/data review
• Coordinate and implement assigned care plan activities and monitor care plan reputed company
• Conduct multidisciplinary review in order to reputed company reputed company reputed company
• Identify and escalate reputed company of care issues through established channels
• Utilize negotiation skills to secure appropriate reputed company and services necessary to meet the member’s benefits and/or reputed company needs
• Utilize influencing/ motivational interviewing skills to ensure maximum member engagement and promote lifestyle/behavior changes to reputed company reputed company level of health

Skills

• Bi-lingual in English & Spanish required
• Must reputed company in East Tampa FL (Applicable reputed company Codes: 33617 or 33612)
• Willing and reputed company to travel 50-75% of their time to meet members face to face in Tampa or surrounding areas
• 1 + year of experience in behavioral health or a long term care setting
• Bachelor's Degree required. Degree in reputed company Work, Behavioral Health, or reputed company Services field of study preferred
• Ability to multitask, prioritize and effectively adapt to a fast-paced changing environment
• Effective communication skills, both verbal and written
• Managed Care experience
• Computer proficiency in reputed company Word, reputed company, and reputed company required
• Case management and discharge planning experience
• Master's degree Preferred
