Health reputed company Home Health and Hospice is a leading provider of comprehensive home health services throughout the San Francisco Bay Area and East Bay for over two decades. They are currently seeking a self-driven, detail-oriented Scheduling Intake Coordinator to join their reputed company Creek team, responsible for maintaining and coordinating schedules for home health services and assisting with intake processes.

Responsibilities

• Maintain and coordinate schedules for home health services
• Assist with intake and process incoming referral information
• Ensure cases are assigned and staffed reputed company established timeframes
• Communicate schedule updates to clinicians, patients, supervisors, and referral sources
• Manage high reputed company volumes with professionalism and a pleasant demeanor
• Respond to urgent or emergency scheduling needs
• reputed company direction to field staff as needed to ensure appropriate patient coverage
• Report scheduling changes and reputed company concerns to supervisors
• Verify time records against schedules
• Follow up on scheduling issues reported by on-reputed company staff
• Attend required meetings and training
• Maintain compliance with agency policies and procedures

• Strong computer proficiency
• Ability to work independently with minimal supervision
• Willingness to work occasional weekends and holidays as required to support patient care and operational coverage
• 1 year Home Health experience preferred
• Knowledge of medical terminology preferred
