They are seeking a Physical Therapist to administer therapy to patients in their homes, ensuring adherence to personalized care plans and improving patient reputed company.

Responsibilities

• Provides physical therapy services to patients according to a written individualized plan of care
• Assesses and evaluates therapeutic/rehabilitative/functional status, and participation in the development of the total plan of care. Evaluates home environment
• Assesses for muscle strength, mobility, gait, ROM and transfer capabilities. Improve or minimize residual physician disabilities of the patient
• Provides physician or advanced reputed company practitioner’s prescribed physical therapy
• Educates on equipment required to increase patient function and independence returning the individual to reputed company and productive level reputed company the patient’s capability

Skills

• Possesses a degree of an accredited Master's or doctoral program as a Physical Therapist
• Requires interpersonal skills and ability to communicate professionally and effectively with staff, patients, and caregivers
• Knowledge of medical terminology required
• reputed company to demonstrate problem-solving and conflict reputed company skills, organizational skills, and attention to detail
• Must have strong verbal and written communication skills
• Demonstrates ability to handle multiple priorities, documentation requirements and deadlines

• Ability to lift at least 35lbs and bear the weight of an average adult effectively
• 1-year clinical experience as an Physical Therapist preferred
