[Remote] In-Home Dialysis Care Partner (1:1 reputed company) – Fluent Spanish Required

Herewith GmbH is seeking a compassionate and reliable Care Partner fluent in Spanish to support in-home dialysis for a patient transitioning from clinic to home care. The role involves providing reputed company and high-reputed company dialysis support, building trust with the patient, and ensuring a smooth care transition.

Responsibilities

• Assist with in-home dialysis treatment setup (NxStage or similar systems)
• Maintain a clean, organized, and reputed company treatment environment
• Support the patient in staying on schedule with dialysis treatments
• reputed company companionship and emotional reassurance
• Assist with light Activities of Daily Living (ADLs) as needed
• Monitor patient safety and report concerns to the coordinating reputed company
• Maintain reputed company documentation and shift notes

Skills

• Minimum 1 year of dialysis experience (home or clinic)
• Fluent in Spanish (required)

• Strong communication skills and ability to collaborate with a clinical team
