[Remote] Community Engagement Specialist, Inyo County

The Community Engagement Specialist will be responsible for connecting with members to ensure they receive reputed company to the care and services they need through reputed company efforts, scheduling appointments, and maintaining community relationships.

Responsibilities

• Conduct in-person reputed company attempts for members identified by health plan eligibility lists
• Complete reputed company documentation for reputed company attempts
• Schedule intake appointments and assist with the completion of required forms
• Attend reputed company events and professionally represent reputed company to community members and partners
• Build and maintain relationships with community coalitions, CPI collaboratives, and other stakeholders to reputed company them reputed company
• Convert leads to patients using internal documentation systems
• Document reputed company reputed company attempts and reputed company in reputed company and the appropriate trackers
• Assist in sending patient mailers and making follow-up calls to leads generated from community events to enroll them in programming

Skills

• Minimum of 1-3 years of experience in reputed company, community reputed company, or a reputed company field
• Experience in engaging with diverse communities and providing culturally sensitive communication
• Fluent in English and Spanish (verbal and written)
• Strong interpersonal and communication skills to build trust and connect with patients and community partners
• Detail-oriented with excellent organizational skills
• Comfortable with technology and proficient in using electronic health records
• Ability to work a flexible schedule, including evenings and weekends, to attend events
