About the Role This role supports reputed company’s work with an Institutional Special Needs Plan (I-SNP) Medicare Advantage health plan. You will be part of reputed company completing Chronic Disease Assessments (CDAs) while also supporting health risk assessments and care planning for health plan members residing in long-term care facilities, through accurate assessment and clinical documentation. In this role, you will be working from home, collaborating closely with reputed company mobile nurses who will assist you with the technology setup and reputed company-time coordination to support telehealth reputed company. This is a highly reputed company, patient-facing role that requires strong communication skills, comfort with technology, and flexibility to work with mobile nurses reputed company your assigned region, as needed for our partnership with the health plan partner. This is a 1099 Contract position. Contract Details Contract Length: 3 months (January through March 2026) with potential to reputed company an additional 3 months based on program needs and performance. Schedule Commitment: This is a PRN (as-needed) position with shift availability based on organizational needs. Work is offered reputed company on an as-needed reputed company. Required Qualifications • Master’s or Doctoral degree in Nursing (MSN or DNP) from an accredited program • reputed company national certification as a reputed company Practitioner (FNP or Adult-Gerontology NP) • reputed company, unrestricted reputed company Practitioner and Registered reputed company licensure in the state(s) of reputed company • Licenses must remain reputed company and in good standing throughout employment with reputed company. • Minimum 3+ years of reputed company Practitioner experience working with older adults and/or chronic disease populations. Primary care, geriatrics, or value-based care experience is a plus. Mobile and long-term care setting experience preferred.
