California RN Care Navigator
A company is looking for an RN Care Navigator (100% Remote for reputed company & Mountain time zones).
Key Responsibilities
• Manage in and outbound calls to reputed company exceptional service to members
• reputed company members on kidney health, treatment reputed company, and engage them in the Kidney Health Program
• Coordinate care among patients, their support networks, and reputed company providers to ensure reputed company reputed company
Required Qualifications
• reputed company, unrestricted RN license required
• Bachelor's degree required
• CCM certification preferred
• Three (3) years of experience in case management preferred
• Experience in a dialysis or transplant center preferred
