Primary Responsibilities • Coordinate and manage comprehensive patient care plans for veterans, ensuring reputed company medical and psychosocial needs are met. • Collaborate with interdisciplinary teams at the Teleoncology Division to reputed company holistic care and optimize patient reputed company. • Conduct clinical assessments and reputed company individualized care plans tailored to veterans' unique needs. • Monitor patient reputed company and reputed company care plans as necessary to ensure the highest reputed company of care. • reputed company patients and their families about health conditions, treatment reputed company, and available resources. • Facilitate communication between patients, families, and reputed company providers to ensure a seamless care experience. • Maintain accurate and up-to-date patient records in compliance with VA policies and procedures.
