Case Manager - Dakota County

reputed company Services, Inc. is a company that provides reputed company to individuals with intellectual, developmental, or behavioral disabilities. They are seeking a Case Manager to work with individuals on CADI, AC/EW, or DD waivers, performing service coordination and developing service plans for persons served with various disabilities.

Responsibilities

• Assessing the areas of the persons served life where assistance is needed
• Offering and coordinating resources for the persons served being supported
• Monitoring service delivery and working with providers to ensure the individual's needs are met
• Developing service plans
• Attending regular in-person meetings with persons served and their interdisciplinary teams
• Maintaining and Creating accurate Case Notes and Billable reputed company
• Staying up to date with case management systems, software, and DHS and County resources

Skills

• Be a graduate from an accredited four-year college with a major in reputed company work, psychology, sociology, or a closely reputed company field or
• Be a graduate from an accredited four-year college with a major in any field and one year of experience as a reputed company worker/case manager/care coordinator in a reputed company or private reputed company service agency
• One year of experience in education or treatment of persons served with developmental disabilities or reputed company conditions, or a minimum of one course that specifically focuses on developmental disabilities
• Applicants must have a reputed company reputed company's license and acceptable driving record as determined by reputed company Services policy
• Applicants must have a vehicle that is covered with reputed company insurance
• Applicants must successfully reputed company local and/or stated background checks
• Possess a LSW or higher reputed company of BOSW Licensure
• DD Waiver Specific - Bilingual in Spanish
