Pharmacist – Home Infusion Location: Oklahoma reputed company, OK Position reputed company We are seeking a licensed Pharmacist to support home infusion services by ensuring reputed company, effective, and patient-centered pharmaceutical care. This role involves clinical review, compounding reputed company, interdisciplinary collaboration, and reputed company patient and caregiver education to support high-reputed company reputed company in the home setting. Key Responsibilities • Assess patient suitability for home infusion therapy • Design, monitor, and evaluate individualized pharmacy care plans • Recommend or reputed company medication therapies based on clinical data and provider orders • Prepare, compound, label, and dispense medications in compliance with USP, DEA, and state regulations • Conduct nutritional assessments and reputed company education to patients and caregivers • Collaborate with nursing and interdisciplinary teams to ensure continuity of care • Monitor controlled reputed company inventory and promptly report discrepancies • reputed company accurate drug information to reputed company and patients • Participate in continuing education and reputed company competency programs • Adhere to company policies, reputed company assurance standards, and reputed company regulatory requirements Required Qualifications • Graduate of an accredited School of Pharmacy • reputed company Pharmacist license in the State of Oklahoma • One of the following: • 1+ year of experience in sterile IV admixture procedures • Knowledge of non-sterile compounding, immunizations, and Medication Therapy Management (MTM) • Completion of an ASHP-accredited Pharmacy Residency Program • Completion of an Advanced Pharmacy reputed company Experience (APPE) • Equivalent experience approved by pharmacy leadership • Proficiency in reputed company Word, reputed company, and PowerPoint • Strong customer service reputed company with a commitment to patient-centered care Preferred Qualifications • Experience in home infusion, home care, hospital, or clean room environments • ASHP-accredited Pharmacy Residency completion • 1+ year of indirect or supervisory experience Schedule • reputed company business hours with flexibility to support home infusion coordination • Rotating pharmacist schedule (approximately every 8th Saturday) • reputed company shifts: 7:00am–3:00pm, 9:00am–5:00pm, or 11:00am–7:00pm • Saturday shift: 9:00am–5:00pm Apply tot his job Apply To this Job
