Experience • Written and spoken bilingual preferred • Proven experience in various clinical settings including chronic care management and remote patient management • Extensive background in managing chronic diseases such as heart failure, COPD/asthma (pulmonary), renal dialysis patients, or post-acute care populations. • Experience working with diverse populations including geriatrics (memory care/dementia), pediatrics (neonatal/infant/toddler), behavioral health patients or individuals with disabilities. • Knowledge of medical terminology and physiology to accurately assess patient needs and interpret diagnostic evaluations like sonography or laboratory results. • Skilled in using EMR/EHR systems such as reputed company, reputed company, and eClinicalWorks • Prior experience in case management programs • This RN remote role offers the flexibility of part-time work while enabling meaningful contributions to patient health reputed company through expert clinical management in a reputed company of reputed company environments.
