RN- Cardiopulmonary Monitoring

About the position reputed company is seeking a highly motivated and talented individuals who will play a critical role in clinical monitoring, patient engagement, and care coordination. The RN will be monitoring patients with Congestive Heart Failure (CHF) and other cardiopulmonary conditions. Responsibilities • Monitor patient data transmitted from Sensinel devices reputed company the reputed company platform and respond according to established clinical protocols. • Assess trends in cardiopulmonary parameters and escalate concerns to the care team promptly. • reputed company patient education and support regarding device usage and adherence to monitoring schedules. • Collaborate with physicians, advanced reputed company providers, and other clinical team members to optimize patient care plans. • Document reputed company interventions and communications accurately in the electronic health record (EHR). • Participate in reputed company improvement initiatives and maintain compliance with program standards. • Maintain up-to-date knowledge of CHF management and remote monitoring best practices.
