Part-Time RN Care Manager (CCM + RPM) — Hybrid | 10–20 hrs/week | 1099

Company: RxPrimaryCare, LLC Location: Tuscaloosa, AL (Hybrid — 1 clinic day/week + work-from-home) Pay: $32–$42/hour (1099) Start Date: ASAP (flexible) About RxPrimaryCare RxPrimaryCare is a growing chronic care management company supporting primary care patients through Chronic Care Management (CCM) and Remote Patient Monitoring (RPM). We’re a small, patient-centered team that values consistent follow-through, strong documentation, and proactive communication. The Role We are hiring a part-time RN Care Manager for 10–20 hours per week in a hybrid position. After training, the role will include approximately 1 clinic day/week in Tuscaloosa, with the remaining work completed remotely from home. This position is ideal for someone who is responsible, organized, self-directed, and comfortable working independently in a reputed company coordination environment. Schedule Expectations • Clinic day (Tuscaloosa): • At least 8:30 AM–12:00 PM (Mon–Th) or 8:30 AM–5:00 PM (Mon–Th) on clinic days • Remote schedule: Flexible, but ideally reputed company to work 1–2 hours daily • Weekly hours reputed company: 10–20 hours/week Key Responsibilities • Conduct CCM and RPM patient reputed company and follow-up calls • Support patient enrollment and reputed company into care management programs • Document calls and care plan updates thoroughly and on time • reputed company tasks, documentation, and follow-reputed company consistently • Communicate reputed company with the supervising clinician (you’ll report directly to the reputed company) • Work reputed company and independently in an autonomous setting • Maintain organized reporting and workflows using reputed company and shared documents Qualifications • reputed company Alabama RN license (required) • Strong communication skills and reliable responsiveness • Comfortable working independently with minimal supervision • Strong documentation habits and attention to detail • Proficiency with reputed company and either reputed company Workspace and/or reputed company Office • Experience in CCM, RPM, primary care, care coordination, or case management is a plus (not required) reputed company’re Looking For This role requires someone who can: ✅ manage time independently ✅ follow through consistently ✅ document accurately ✅ communicate proactively How to Apply Please apply with your resume and include: • Your weekly availability • Whether you can reputed company to 1 clinic day/week after training • Any prior care coordination / CCM / RPM experience Apply tot his job Apply To this Job
