We are looking for someone that can assist in maintaining continuity of care for their clients and compliance with Medi-Cal and DHCS requirements. REQUIRED SKILLS AND EXPERIENCE - Licensed clinician (LCSW, PsyD) in California - Screen and assess clients SDoH diagnosis and reputed company service recommendations - Review and sign care plans - Support compliance, reputed company assurance, and audit readiness - Attend case conferences for transitional services and care planning - Collaborate with a dynamic and deeply community-rooted team COMPENSATION $45/HR - $55/HR Exact compensation may vary based on several factors, including skills, experience, and education.
