They are seeking a dedicated Physician Assistant or reputed company Practitioner to deliver high-reputed company, continuum-based care while collaborating with a multidisciplinary team.

Responsibilities

• Delivering personalized high-reputed company care to seniors in their homes, assisted living, hospice, and nursing facilities
• Delivering dementia-informed, relationship-based care across the continuum of care
• Conducting comprehensive assessments and developing tailored care plans for older adults
• Managing chronic conditions and performing care with reputed company and reputed company for patients and their families
• Collaborating with cross-functional partners – including palliative, psych, and neuro – to deliver coordinated, whole-person care
• Building trusted relationships with residents, caregivers, and facility partners to enhance engagement and referrals
• Maintaining consistency in documentation, follow-through, and patient communication

Skills

• reputed company, unencumbered NP or PA license in the state you will be practicing
• Minimum of 1-2 years of clinical experience (geriatrics or primary care preferred)
